Younger adults and disabilities services

What do we do?

The Younger Adults and Disability Services Team (YADS) are a team of qualified social workers who work with people between the ages of 18 and 65. We will arrange services for people who need support, assistance and advice who:

  • are physically disabled
  • suffer from a long term condition or illness
  • are unable to live independently due to other problems
  • care for a person with one or more of the above

Our aim is to give you the support that you need to help you look after yourself at home and to help you stay as independent as possible.

Types of help

We can arrange a number of services including:

  • assistance within your home – equipment to help you with day to day tasks, adaptations to your home
  • support and advice with all aspects of daily life – getting out and about, employment, activities
  • support, advice and information about social security and welfare benefits
  • Direct Payments
  • daytime support in a day centre
  • rehabilitation
  • a move to a specialist care home

How do I get help?

If you think you need help from us you can ask for advice or an assessment.

You can contact us by calling the YADS social work team direct.  Our staff will give you advice and information.  We may need to see you to make a decision about this.  This is called an assessment.  If your situation is urgent or you are at very serious risk of harm, we will respond immediately, otherwise we will aim to start the assessment within 2 days.

After you contact us, one of our staff may arrange to meet with you to talk with you about yourself and what you feel you need help with.  This is called an assessment.  We will ask you questions like:

  • What can you do for yourself?
  • What do you need help with?
  • What do you need to have done?
  • Does anyone help you or look after you at the moment?

You are welcome to have a relative or friend at the assessment.

Your carer can also have a separate assessment for their needs, if they wish.

It does not cost anything to have an assessment.

Your social worker may also ask health colleagues or other people who work with you to contribute to the assessment to make sure all of your needs are included.

After talking to you, we will work out what help we may be able to give to you.  

Once we have agreed with you what we can help you with, we will talk to you about how you want your needs to be met.  All of this will be agreed between you and the social worker, and will be written down in a care plan.  We will give you a copy of this plan.

If we cannot help you we will give you details of other agencies or organisations that may be able to help you.

How much will these services cost?

You may be asked to pay for some of the help we give you.  How much you pay will depend on your income and the amount of help you need.

When we come to talk about your needs we will also talk to you about your finances.  We will ask you questions about your income, your savings and how much you spend each week or each month.

This will help us to work out how much you have to pay.  We will explain any charges to you and tell you how we have worked them out.  For more information on what our services cost, contact the public information officer on 01922 658368 and ask for a copy of our ‘Fairer Charging’ leaflet.

If you are getting help from social services in some cases you can choose to have the money to pay for these services and choose who you want to carry out the services for you. We can also help you to find out if you are able to claim any benefits.

You will not be charged for any health services if they are included in your care plan.
