Long-term care charter
This charter is for people over 18 who have difficulties because of old age or a long-term physical or mental illness or disability. It is also for those people’s carers. In this charter we talk about community care services.
Community care services are support services that we may be able to give you to help you look after yourself at home and to help you stay as independent as possible.
