Important application instructions
- A separate nomination form must be completed for each category.
- Nomination forms that highlight more than one category will be disqualified.
- Please complete the nomination form with as much information as you can.
- Please ensure that your applications are returned by Friday 19 December 2008.
- If you have difficulty in understanding the form please telephone 01922 652426 leaving a message in your own language. Please provide your telephone number and contact address so that we can respond to you.
What happens after the deadline?
- Following the deadline date of 19 December 2008, shortlisting will take place. If your nominee is a finalist we may contact you for further information and details of a second referee.
- All nominations will be considered by an independent panel.
- The panel’s decision is final.
- We regret that we will only be able to contact those who have been shortlisted.
Walsall Council is collating information for the sole purpose of evaluating nominations for the Mayor’s Civic Awards.
Information provided on the nomination form will only be disclosed to relevant council officers dealing with the Awards scheme and members of the Evaluation Panel. The names of finalists, reasons for their nomination and the award category will be disclosed to the press and other media.
You should note that applications for the Awards will be kept for a period of three years.
