If you wish to proceed with your complaint, please download the complaint form and complete all of the sections that are relevant to your grievance.
- Section 1 - Provides us with your details.
- Section 2 - Provides us with details of the name of the councillor you believe has breached the Code of Conduct.
- Section 3 - Provides us with an explanation of why you believe the councillor has breached the Code of Conduct.It is important for you provide as much information as possible in this section so that the Assessments Sub Committee can decide what action to take regarding your complaint.
- Section 4 -Only complete this section if you wish your identity to be kept private along with an explanation as to why this information should be withheld.
- Section 5 - Gives us details of your complaint type.
- Section 6 -Provides us with equality monitoring data.
