Help for people with learning disabilities who are leaving school
The Transition Team provides support and information to young people with moderate, severe or profound learning difficulties during their transition from school to adult life.
The Transition Team are part of the Walsall Integrated Learning Disability Team. The team is made up of a social worker, a community nurse and a transition facilitator.
What we aim to do
- To support young people with a learning disability who are leaving school and help successfully make the transition into adult life
- To support families and carers during this time
- To commission local services to meet the specialist needs of young people in transition
- To work with specialist and community agencies
- To use the transition pathway to ensure best practice
- To offer every young person with a learning disability a community care assessment and health action plan
Who do we support?
We support young people with a moderate or severe learning disability from 16 years old. These young people's care and funding of their care packages are still the responsibility of children's services until they are 18 years old.
We support individuals who have gone straight from school to residential colleges and are now ready to leave.
We will work with them until they are 20 years old, unless they have a stable care package before this time. If more support is needed after this, we will refer the person to the appropriate learning disability team.
